Sacramental Record Request

If your sacramental information is over 70 years old, or if the event took place at a currently closed parish, it is most likely that your sacramental information is held at the Archives & Records Center.

In order to see if your sacramental information is currently at the Center, check the listing of Sacramental Records Currently at the Archives & Records Center.

If your sacramental record is here, fill out Sacramental Record Request Form and mail it to the Archives & Records Center, along with a self-addressed stamped envelope.

If you are a parent of a minor child (who is under the age of 18) who needs a copy of their sacramental records, fill out the Parent/Guardian Sacramental Request Form and mail it to the Archives & Records Center, along with a self-addressed, stamped envelope.

Anyone can obtain a copy of their own sacramental records free of charge.
